ORANGE COUNTY

In 2014, the City of Anaheim asked us to replicate our proven impact from the City of Long Beach in Anaheim. In Orange County, we provide a variety of outreach and engagement services and operate programs including census coordination, housing navigation, shelters, and street outreach (7 days a week) in many of the cities we partner with. We are proud of the collaborative work we perform in Orange County.

CURRENT ORANGE COUNTY SERVICES

Street Outreach Logo
  • Street outreach is a way to build rapport and trust with people experiencing homelessness, utilizing whole-person care, mental health and behavioral health first aid, trauma-informed care, harm reduction, housing first principles, de-escalation, motivational interviewing, and progressive engagement.

Case Management Logo
  • Case management activities assess housing and service needs and arrange, coordinate, and/or monitor the delivery of individualized services. Specific activities include:

    • Assessment and intake using HMIS

    • Conducting initial evaluations including verifying and documenting eligibility

    • Counseling

    • Developing/ securing/ coordinating services

    • Helping obtain Federal, State, and local benefits

    • Monitoring and evaluating program participant progress in particular programs

    • Providing information and referrals to other providers

    • Developing an individualized housing stabilization plan (see also the housing preparation section below) that leads to the attainment of stable permanent housing.

Unhoused Census Logo
  • An unhoused census is a systematic process of gathering, analyzing, utilizing, and presenting data to understand and address various aspects of those experiencing homelessness.

    Our censuses are a distinct but complementary project from the HUD-mandated, county-wide Point-in-Time Count. The census allows for a city-specific focus with more time, staff resources, and local questions than is generally possible within the framework of the Point-in-Time Count.​

    We provide comprehensive data on the unhoused population within a city including but not limited to population counts, heat maps, demographics, and other customizable dat sets. If you are interested in contracting with us, please contact census@citynet.org.

Dispatch Response Logo
  • Dispatchers direct calls from the community and law enforcement to our Multi-Disciplinary Response Teams for all non-violent homeless service requests. Our teams respond to these calls within 15 minutes and begin engagement in services with the unhoused in need.

CalAIM logo
  • City Net began CalAIM services in 2023, where we connected persons experiencing homelessness to housing through state funds from the Department of Health Care Services that did not cover the costs for supportive services or move-in assistance. The CalAIM Community Supports programs allow us to supplement our contracts to immediately serve our clients in need to provide Housing Transition/Navigation Services, Housing Deposits, and Housing Tenancy and Sustain Services (the “Housing Trio”).

Housing Navigation Logo
  • Housing Navigation provides end‐to‐end support—conducting needs assessments and budgeting, completing applications and securing required documentation, overcoming credit or legal barriers, and facilitating introductions to landlords and neighborhoods—while case managers address housing histories, family dynamics, and other readiness factors to successfully match clients with stable housing.

Bridge Housing Logo
  • The Behavioral Health Bridge Housing (BHBH) Program, funded by the California Department of Health Care Services (DHCS), helps us operate bridge housing settings to address the immediate housing needs of people experiencing homelessness who have serious behavioral health conditions, including serious mental illness (SMI) and/or substance use disorder (SUD).

    The BHBH Program will be implemented in alignment with the Community Assistance, Recovery, and Empowerment (CARE) Program, which prioritizes BHBH Program resources for CARE participants. 

ORANGE COUNTY CURRENT PROJECT DASHBOARDS

  • This program, which serves the entire County of Orange, is enabled by funding from the California Department of Health Care Services.

    It provides 24-hour wraparound housing and Case Management to 50 high utilizers with Severe Mental Health issues.

  • Beginning in 2021, this innovative program was the second of its kind in Orange County, furthering a partnership between the City of Santa Ana, the Santa Ana Police Department, and City Net. All nonviolent service requests for unhoused people are redirected to our dispatchers who deploy multi-disciplinary response teams to provide a safe and quick response to some of our most vulnerable neighbors.

  • Beginning in 2021, this innovative program was the first of its kind in Orange County, pioneering a partnership between the City of Anaheim, the Anaheim Police Department, and City Net. All nonviolent service requests for unhoused people are redirected to our dispatchers who deploy multi-disciplinary response teams to provide a safe and quick response to some of our most vulnerable neighbors.

  • This initiative coordinates Street Outreach and Engagement with the Fountain Valley Police Department’s HLO (Homeless Liaison Officer) for those experiencing homelessness in Fountain Valley.

  • This initiative coordinates Street Outreach and Engagement with the Tustin Police Department’s HLO (Homeless Liaison Officer) for those experiencing homelessness in Tustin.

    Services include referrals to all mainstream benefits, identification documentation, housing needs, medical & mental health services, substance abuse services, and access to food and shelter.

  • This initiative provides Outreach and Engagement to Union Pacific Railroad property for those experiencing homelessness throughout Southern California.
